Thrown when an impersonation transition is refused — the principal holds no active delegation for the requested target, or a grant would violate a structural invariant (self-delegation, overlapping active window).
The Intent path never lets this surface: the dispatcher's authorize() gate denies first (fail-closed). It guards the domain API for out-of-Intent callers.
